· Contacting the conference secretariat to receive a copy of the form via fax or email.

 

Please Note:Some of the amounts under "Exc GST" on the online registration form differs from the hardcopy you received in the mail.

N.B. If you have problems in downloading the form please contact the Conference

Secretariat via email so she can send you a copy in word format.
